A claim submitted incorrectly will be rejected and not paid, sometimes for months! A claim can be rejected because the employer’s name was omitted. A claim can be rejected because the procedure is not covered at the doctor’s office but IS covered as a referral. There are many reasons claims are rejected, it is the responsibility of the biller to make sure a claim is properly submitted the first time. Even a lot of the doctors aren’t familiar with the correct codes for different procedures. To clarify further, an employer buys coverage for their employees through an insurance company. The insurance company customizes the coverage based on the employer’s needs. Let’s say you work for employer A and your sister works for employer B. You both have the same insurance company, but the coverage can be totally different between the two. The medical biller would need to know the specific coverage for each employer to properly submit claims.”

“It’s possible but only after hands on experience and whether or not a physician would allow it. The majority of patients are covered through an HMO or PPO plan. A physician would have to be very comfortable with your knowledge and ability to submit claims before they allowed you to work from home. Let’s face it, the medical biller controls the income. High denial rates cause hardship on the doctor’s practice. The claim has to be accurate, submitted correctly the first time and paid in a timely manner for the doctor to stay in business. Hands on training is the only way you will learn the individual physician’s network. An experienced medical biller would probably have to spend less time in a new job updating and training before they could work at home. Physicians should monitor new employees in the beginning. They can’t afford to find out 30 days later that a majority of their claims were rejected, it takes much too long to resubmit.”

All in all, spend time researching a medical billing course and make sure you are getting the best training you can afford. Inexpensive classes often don’t teach much more than data entry. Realize you are most likely going to be required to spend onsite time before you can work at home. Medical coders are in demand so the market is certainly a good market and not evaporating. You have time to research and do your homework, it just isn’t as easy as those advertisements lead you to believe!

Google-Motorola deal highlights fierce patent arms race

NEW YORK — When an Internet company plunks down US$12.5 billion to buy a struggling cellphone company for its collection of patents, it's another sign that, for the high-tech industry, patents have become a mallet wielded by corporations to pummel their competitors.
Google Inc. announced the deal to buy Motorola Mobility Holdings Inc. on Monday, specifically for its trove of 17,000 patents. Google needs them to shield companies like HTC Corp. and Samsung Electronics Co. - who make phones based on Google's Android software - from lawsuits filed by Microsoft Corp. and Apple Inc.

"Google is not acquiring Motorola for the sake of its technology or its research," said James Bessen, a lecturer at Boston University and co-author of a book on the patent system. "Patents have become legal weapons - they're not representing ideas anymore."

The trend, decades in the making, raises questions that pending patent legislation in Washington only begins to answer.

Google's multi-billion bid to get its hands on Motorola's output of legal paperwork is the culmination of a "bubble" in the value of patents relating to smartphones that started last year, as Microsoft and Apple mounted their legal attack. Industry watchers say that bubble may deflate now that Google is set to gain the protection of Motorola's patents in a deal that's set to close late this year or early next.

But an underlying problem will keep growing: patent filings and lawsuits that distract companies and sap resources that are better spent on other things.

Engineers spend their time writing patents rather than inventing things, or reworking products just to avoid patent infringement. Customers put off purchases because of pending lawsuits, and independent software developers close up shop because they can't afford licensing fees.

"If you have to pay $12.5 billion dollars to play, you can sense why maybe an individual who has a great idea would feel discouraged," said Julie Samuels, a patent lawyer with the Electronic Frontier Foundation, a technology-oriented civil liberties group. "It affects the whole economy."

It wasn't always this way. The U.S. software industry got its start with nary a patent filed, and on the hardware side, patent suits were rare until the mid-1980s. That was when calculator and chip maker Texas Instruments Inc., on the brink of extinction, decided to see if it could make some money from its patent portfolio. It started filing patent lawsuits and demanding money from companies with infringing products. It saved the company.

IBM Corp. latched on to TI's lead in patent licensing in the mid-90s, when it was down on its luck. That coincided with courts broadening the types of patents allowed. Patents on software and "business methods," with vague, broad claims, were now accepted.

Since then, an arms race has slowly escalated in the industry. Companies found that the best defense against a patent suit from a rival was to have a patent portfolio to wield as a deterrent: "Sue me and I'll sue you back," is the message Google is sending by buying Motorola.

Motorola is already suing Apple over several patents, including one that purports to cover the act of sending address data between two phones. Another patent at issue covers the idea of concealing a phone's antenna in its outer case, which Apple arguably does with the iPhone 4.

It's a situation reminiscent of the nuclear standoff between the U.S. and the Soviet Union. But just as the threat of nuclear weapons didn't stop third-world guerillas during the Cold War or deter terrorists today, the patent arsenals are useless against "patent trolls" - companies that own patents but don't do actual research or development. Since they don't make anything themselves, they can't be the targets of patent suits, says Colleen Chien, assistant professor at the Santa Clara University School of Law.

"Mountains of patents have proven useless against the patent system's 'stateless actors,' non-practicing entities who are invulnerable to patent counterclaims," Chien writes.

In one example, a company with a 1980s patent on a kiosk that made music audiotapes on the spot for customers in stores tried to levy license fees from tens of thousands of technology companies, claiming that the patent covered any downloading of media from the Internet. Microsoft was among the companies that settled.

Bessen puts the cost of dealing with "patent trolls" at half a trillion dollars in the last two decades. Yet patent trolls account for only one in six patent suits, by his estimate, so the patent system's burden on the economy is much higher.

Just as we worry about old Soviet nuclear weapons falling into the wrong hands, Chien says that the patent hoards accumulated by corporations as "defensive" measures are starting to end up with "non-practicing entities" who use them for lawsuits.

For example, memory chip-maker Micron Technology Inc. in 2009 sold 4,500 patents to a patent lawyer in 2009. Chien points out that the patents are worth more to "non-practicing entities," because they can sue without fear of retaliatory patent suits.

During the Cold War, there were arms limitation talks. Similarly, many of the big technology corporations want the patent bombs taken away, or at least limited. Google's lawyers are critical of the patent system, and it's clear the company would rather not have to strike deals like the one to buy Motorola. Cisco Systems Inc., the world's largest maker of networking gear, wants patent infringement damages to be based on the value of the component in question rather than the entire product.

Tech companies can expect little help from Washington. After a decade of wrangling, Congress is set to approve a patent reform bill when the Senate reunites in December. It will be the largest legislative change to the patent system since 1952. Even so, experts say its effect on the high-tech industry will be marginal. It had sought more sweeping changes, but resistance from the pharmaceutical industry, which is much better served by the current system, has kept out the more radical proposals.

The legislation will make it marginally harder to get and hold onto patents, Chien said, but that's unlikely to cut down on the number of spurious patents, she believes.

And paradoxically, the bills could expand the glut of patents that's plaguing the industry, since one of its goals is to reduce the three-year backlog of patents pending at the Patent Office.

"It's going to take a long time for Congress to tackle patents again, and that's really a problem because this troll problem is going to continue to fester," Samuels said. "We all feel the effects."

Phone makers depending on Google Inc's Android software should worry about the Web search leader's deal to buy Motorola Mobility, the head of rival Nokia said on Wednesday.
Nokia has teamed up with Google arch-rival Microsoft for mobile phone software, while phone makers such as Samsung Electronics, HTC Corp and Motorola have bet on Android.

But Google's plan to buy Motorola for $12.5 billion has generated some analyst concern about whether Motorola will get preferential treatment over rivals Samsung and HTC. Nokia Chief Executive Stephen Elop said these concerns may be justified.

"If I happened to be someone who was an Android manufacturer or an operator, or anyone with a stake in that environment, I would be picking up my phone and calling certain executives at Google and say 'I see signs of danger ahead,'" Elop told a Helsinki seminar.

As for Nokia, Elop appeared to suggest that Google's move reinforced the logic for Nokia's agreement with Microsoft

"The very first reaction I had was very clearly the importance of the third ecosystem and the importance of the partnership that we announced on February 11, it is more clear than ever before," Elop said.

He also said that since the February announcement of the deal with Microsoft, there are now 25,000 to 30,000 applications delivered for the upcoming platform.

Before Elop become CEO of Nokia last year, he was president of Microsoft's Business Division.

Shares of rivals Research In Motion and Nokia jumped by about eight and 16 per cent, respectively, as traders speculated that takeover offers for those two companies, which both have rich patent portfolios, could also attract premium bids.
National Bank Financial analyst Kris Thompson estimated that RIM's patents could now be worth $10 billion and said speculation on their value could lift RIM's share price in the short term.
“We believe Google’s acquisition of [Motorola Mobility] will set a floor-value for RIM’s shares around current levels,” Thompson wrote in a research note.
The deal also marks a major turning point in Google's strategic plan. Until now, it has largely confined itself to software development. Now, it also becomes a major manufacturer of telecom hardware.
What the deal won't change, according to Google, is the company's commitment to Android as an open platform.
"Motorola will remain a licensee of Android and Android will remain open," said Page. "We will run Motorola as a separate business."
Motorola Mobility was split off from the rest of Motorola in January. Motorola made the first commercial portable cellphone in 1983.
Motorola Mobility's shares soared $13.63 to $38.10 US Monday on news of the takeover offer, which requires regulatory approval.
The biggest shareholder beneficiary of this deal is billionaire activist investor Carl Icahn, who owns more than 11 per cent of Motorola Mobility. He had long urged Motorola to sell off its valuable patents.
Some analysts said Google's expansion into the cellphone manufacturing world could complicate relations with some Android partners who will suspect that Motorola will get a better deal from its new parent.
"Google will move from the position of partner to that of competitor to Android handset manufacturers, potentially placing significant strain on the Android ecosystem," Ovum analyst Nick Dillon wrote in a Monday note. He thinks some cellphone and tablet makers will switch to other software platforms if they detect the slightest evidence of favouritism.
But Google CEO Page pledged to treat all Android partners fairly. "Many hardware partners have contributed to Android’s success and we look forward to continuing to work with all of them to deliver outstanding user experiences," he wrote.

Google is buying cellphone maker Motorola Mobility for $12.5 billion US in a blockbuster deal that will see the search engine giant acquire Motorola's vast array of patents and turn Google into a mobile-phone maker.
The deal is by far Google's biggest acquisition to date. It will pay $40 per share in cash, a huge 63 per cent premium to Motorola's closing price on Friday. The $12.5-billion price will consume almost a third of Google's cash hoard.
Google makes the popular Android operating system that powers about 150 million cellphones around the world.
Motorola is one of dozens of manufacturers that makes cellphones that run on the Android platform, in direct competition with Research In Motion's BlackBerry, Apple's iPhone and Microsoft's Windows mobile operating system for smartphones.
"Motorola Mobility's total commitment to Android has created a natural fit for our two companies," Google CEO Larry Page said in a statement. "Together, we will create amazing user experiences that supercharge the entire Android ecosystem for the benefit of consumers, partners and developers."
There's no question that a driving force behind Google's purchase is Motorola's valuable cache of 17,000 patents on phone technology. It has more than 7,000 patent applications pending.

SEOUL - Samsung Electronics chairman Lee Kun-hee has asked the firm's top management to seek measures including acquisitions to enhance its software prowess, local media reports said on Wednesday.
His remarks come as Samsung is embroiled in an intensifying patent battle with Apple, and after Google unveiled a $12.5 billion US deal on Monday to buy Motorola Mobility Holdings, threatening its ties with the Android mobile software platform maker.
"Chairman Lee told top managers to come up with various measures including M&As to enhance software competitiveness," Kim Soon-taek, head of the Samsung Group office, told local reporters.
Lee made similar remarks in late July, urging managers to boost its software prowess, patent pools and talent at the firm's annual event comparing Samsung's vast product portfolios ranging from televisions, handsets and computers against those of its rivals.
At the time, Lee stopped short of mentioning mergers and acquisitions.

Samsung is the most credible challenger to Apple in the booming smartphone and tablet market and Apple is moving aggressively to defend its dominance in those markets it virtually created.

Analysts say that the South Korean firm faces the challenge of moving beyond being a hardware company, clever at copying ideas, to become more creative, better adept at software, at a time when consumer gadgets are getting smarter all the time.

When Katrina Kaif  failed to blow whistles for a sequence in her item song in “Bodyguard”, guess who came to her aid? Her former beau Salman Khan!
Katrina, who features in the second half of the song “Aaya re aaya bodyguard’, was uncomfortable when she was asked to blow six whistles in the song. She couldn’t whistle at all!
But Salman helped her out, tells a source from the sets.
In fact, it was Salman, who blew the whistle for Katrina in the song. The two continue to share a warm relationship despite parting from each other.
“Bodyguard”, which features Kareena Kapoor as the leading lady, releases Aug 31.

It seems Sanjay Dutt’s favourite genre is comedy — he returns in a comic avatar in his long delayed film “Chatur Singh Two Star” to tickle the funny bones of the audiences after entertaining them with “Double Dhamaal”.
Directed by Ajay Chandok and co-produced by Pen India Pvt. Ltd and Lotus Pictures, the film, which also stars Ameesha Patel and Suresh Menon, Anupam Kher, Shakti Kapoor, Satish Kaushik in prominent roles, is releasing Friday.
The shooting of the film was completed in 2008 and it was planned to be released in 2009, but was delayed due to market conditions. And Sanjay is hoping that the film works well at the box-office.
Made at a budget of Rs. 25 crore (approximately), it will be released on 700-800 screens across India. A blend of comedy and action, “Chatur Singh Two Star” was shot in Cape Town, South Africa, and Mumbai. The music of the film is being composed by Sajid-Wajid.
The satellite rights for the film have been sold for Rs.8 crore.
The film is about a clumsy cop Chatur Singh who botches up cases and his goof-ups embarrass the police department. Despite his clumsiness, Chatur Singh embarks on an important investigation and jets off on a secret mission to South Africa along with his right-hand man Pappu Panther (Suresh Menon). This is a lifetime opportunity for him to prove himself.
Both of them bump into Sonia (Ameesha Patel) and with her help, Chatur Singh succeeds in finding the right clues and while doing so he kicks off a string of hilarious events.
The small budget movie is based on novel “Chalaak Jasoos” written by a Nagpur-based writer, M Raza. And Sanjay even changed his look for his role. The novel describes the main character as a clumsy guy with a funny moustache and the actor will be seen sporting something similar in the film.
Sanjay also put on weight to fit into the character.
The actor not only worked on his look but has also sung a song for the film. After singing for “Munnabhai” series, Sanjay has crooned two songs in the film — “Murga anda dega” and the title song “Singh Singh Singh…Chatur Singh Coming”.
Rumours were abuzz that the film is inspired by “The Pink Panther”, however, Sanjay denied any similarity between the two films.

A key consideration is the role of ADR in the resolution of business disputes. The committee heard a presentation from Deborah R. Hensler, Judge John W. Ford Professor of Dispute Resolution and Associate Dean for Graduate Studies at Stanford Law School.  Based on her reading of the empirical evidence, Hensler is a skeptic about the efficacy of case management techniques, asserting that “we know less about
the effects of case management than some judicial reformers claim” and that “some of the case management approaches that U.S. experts sometimes recommend to courts … may not have the consequences the experts suggest they will have.

In law and economics, insurance is a form of risk management primarily used to hedge against the risk of a contingent, uncertain loss. Insurance is defined as the equitable transfer of the risk of a loss, from one entity to another, in exchange for payment. An insurer is a company selling the insurance; an insured, or policyholder, is the person or entity buying the insurance policy. The insurance rate is a factor used to determine the amount to be charged for a certain amount of insurance coverage, called the premium. Risk management, the practice of appraising and controlling risk, has evolved as a discrete field of study and practice.
The transaction involves the insured assuming a guaranteed and known relatively small loss in the form of payment to the insurer in exchange for the insurer's promise to compensate (indemnify) the insured in the case of a financial (personal) loss. The insured receives a contract, called the insurance policy, which details the conditions and circumstances under which the insured will be financially compensated.

It seems like Apple would release two generation of its tablet in a single year, though it seems improper from Apple this is also true that a part of us really wanted to believe it will happen. Yet, the Wall Street Journal just took stage and killed whatever hopes we had left for seeing a Retina display-packing iPad 3 this year.

According to the publication, the Cupertino-based company is aiming at starting trial-production of the iPad 3 in October, so the official launch cannot realistically come sooner than the start of next year. So we should be getting the iPad 3 and its sweetly sounding 2048 by 1536 pixels display no earlier than 2012. And if you think about it, the difficulties manufacturing the high-res screen might not be only thing that’s holding it back.

With the resolution quadrupled, the next iPad will probably need at least twice the processing power of the current edition to deliver the same performance. And, except for the Nvidia Tegra 3, there probably won’t be any other mobile chipsets with quad-core CPUs available until the start of 2012. Unless Apple has a new chip up its sleeves.

HTC is one of those Smart Devices Manufacturer who are Major Players on Android-Powered Devices Market but still HTC has not fully entered the tablet battle – the Taiwanese giant is yet to announce a tablet, running on Android Honeycomb. It looks however, that the launch of the device is nearing as today, we stumbled into more screenshots of the company’s yet to be announced, first Android Honeycomb tablet, which is known by the name HTC Puccini.
Unlike the last time we saw screenshots of the upcoming device, it is now clear that HTC has managed to implement its wildly popular Sense UI into the Android OS. Logically, this means that along with the familiar Android functionality, we can expect to see a lot of eye candy on the tablet’s 10″ screen.

We already know that the HTC Puccini will be launched in the United States by AT&T under the name HTC Jetstream. It will be one of the first devices to run on the company’s newly built LTE network. The tablet’s CPU is expected to be a 1.5 GHz dual-core unit, which will power some of the company’s Android phones like the HTC Ruby.
